Church of St. Augustine Parish Registration Form

We welcome all new members to our parish, and kindly ask that you register, so we may most effectively serve you. Please fill out the form as found below.

We also encourage you to become an active member of the parish by getting involved in one or more of our varied ministries. You'll have an opportunity to find out more about parish ministries after you complete this form.

Tithing is also an important aspect of parish life, and is usually calculated as 5% of your total income. Stewardship is your decision in prayer. Should you choose to support the parish financially, an opportunity to do so will be provided after you complete this form.

Going forward, please alert the parish office to any change in your address, so we may keep accurate records.

This registration is our only means of certifying anyone as a member of our parish for Baptism, Confirmation, and Wedding purposes.
